Your Physician
The patient remains under a doctor’s care. Madison coordinates the home health plan with the physician or specialist.

Beyond Madison’s team
Coordinating other providers when the plan needs more
Some patients need services or equipment beyond what a home health team directly provides. Madison’s current site describes coordination with outside resources such as home medical equipment, home pharmacy and medical transportation.
The goal is to keep the different parts of care connected, while the patient remains involved in understanding the plan.
